MISSION STATEMENT
The purpose of the Green Brook Township Public Schools Athletics Program for grades 6 – 8, with the exception of cross country (grades 5-8) is to initiate, stimulate, and improve opportunities for students to expand their learning through competition; and to promote and develop leadership, sportsmanship, fitness, school spirit, and athletic excellence through mass participation in various athletic disciplines.

SPORTS MEDICAL FORMS DUE DATES
NJ MEDICAL SPORTS FORMS
Physical examination is required once per year within the last 365 days and prior to the form’s deadlines. A new MEDICAL ELIGIBILITY FORM must be submitted to the GBMS main office within two weeks of the expiration date should your child’s physical expire before the end of the season.
The following medical forms need to be completed by clicking the links below:
- PHYSICAL EXAM & MEDICAL ELIGIBILITY FORMS The physical exam & medical eligibility forms should be completed & signed by a physician. ONLY the Medical Eligibility Form needs to be submitted online/hard copy to the GBMS main office. Physical Examination Forms should be kept by the healthcare provider who completes the PPE.
- MANDATED ATHLETE INFORMATION FORMS (completed once per year & submitted electronically! )
- HEALTH HISTORY FORM The Health History Update Form can be submitted online/hard copy to the GBMS main office. To participate on a school-sponsored interscholastic athletic team, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update completed and signed by the student’s parent or guardian.
