Sports Participation Form Directions
Submission of Sports Permission Forms:
All forms must be completed and submitted to the school nurse prior to the due dates listed below for the student’s participation in a sport.
Incomplete or missing forms will not be accepted for sports participation.
It is the responsibility of the student and his/her parents to make sure that all forms are competed and delivered to the school prior to due dates.
NO LATE forms will be accepted past the due date, including physical examinations.
Sports Participation forms due dates are:

Sport Due date Soccer, Cross Country Sports August 1st. Basketball October 15th. Softball, Baseball, Track and Girls on the Run February 15th.
Initial Sports Participation Forms are all the forms that are needed for a student to initially participate in interscholastic sports; ie. soccer, cross country, basketball, softball, baseball, track and Girls on the Run.
These multiple forms must all be completed for a student to begin his/her participation in an interscholastic sports.
The forms include:

- # 1 Parental Permission Form (front and back) – (parental read, complete and sign, student read and sign)
- #2 Health History Part A – (parental completed and sign)
- #2 Physical Exam Part B – (parental complete top only, health care practitioner complete and sign)
- #3 CDC Concussion Information Form – (student and parent read and sign)
- #4 IMPACT Testing Permission Form – (parental read and sign)
- #5 Sudden Cardiac Death Information sheet – ( no signature required)
These forms with a physical exam must be competed once every school year.
The Physical Exam (Part B) must be completed by your health care practitioner less the 365 days before the start of the sport season. If during the sport season the physical day date is more than 365 another physical will be needed within two weeks after the one year expiration date of the last physical for the student to continue participating in the sport.
Secondary Sports Participation Form # 6:
This single sheet (salmon colored) form is utilized ONLY for participation in an additional sport, during the school year, once the packet of the “Initial Sport Participation Forms” has been completed and if the initial physical exam is less than 365 days old.
